FEES AND PAYMENTS

The receptionist can give you information regarding fees for services. We will make every effort to keep down the cost of your medical care. If you anticipate any problems in paying for all of the charges, please discuss this with the receptionist and she can work with you in arranging a payment plan. Personal checks and cash are accepted, and payment at the time of services for fees is expected.

INSURANCE AND INSURANCE FORMS

We try to simplify the preparation of insurance claims and thereby hold down the costs that are unrelated to the delivery of good medical care. Please be certain that the receptionist has all the appropriate information, especially your identifying infromation and your current address. Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurance or any other balance not paid by your insurance company. We know questions can arise on insurance matters, and these should be discussed with our insurance clerk. We will be happy to help you receive maximum benefits; however, the agreement of the insurance company to pay for medical care is a contract between you and the company.

MEDICARE

As a service to our Medicare patients, we will file your Medicare claim directly if Medicare is your primary insurance. This office accepts assignment on all Medicare claims. The patient is responsible for the 20% of the allowable amount and for the yearly deductible. The patient may have a co-insurance to help out with the copay and the yearly deductible. Please be sure that the receptionist has all of the appropriate information and your current address.

HOSPITAL INSURANCE

If you are hospitalized and have hospital insurance, our insurance clerk will send you a universal insurance claim form that you will be able to attach your completed form and forward on to your insurance company. If you have questions about our policy please feel free to call our insurance clerk. Again, insurance is a contract between you and the company.
